Vulnerable Population Teaching Project: Final Report: Teaching Plan for Diabetes Management in Brooklyn NY Directions: You performed a community assessment and identified a vulnerable population that you wanted to work with. You developed a teaching project proposal based on the identified health risks and educational needs of this population. This assignment is the final report of the implementation of the teaching project that you started in previous class. Note: Your submission should also include the evaluation tool that was developed to determine whether the learning objectives were met. This report should be submitted as a formal APA paper complete with a title page citations and references. The paper should be approximately 8-10 pages not counting the title page references and appendices. Please use the attached template to write the final report of your teaching project. The paper should include the following sections: Requirements: 8-10 pages Introduction (5%) (One or two paragraphs briefly highlighting the main points of the project and introducing each section of the paper.) Background (5%) (Brief summary of the community assessment that was performed in NURS421 and the rationale for why this vulnerable population was selected for this teaching project). Literature Review (20%) (This should be a substantial review of the relevant scholarly studies that support the need for this teaching project. There should be a minimum of 5 references from scholarly sources. ) Description of the Teaching Project (30%)This section should include the following sub-sections: Learning objectives (The objectives must be written as behavioral objectives that are measurable. An example of a measurable objective would be: At the end of the teaching session the participants should be able to: List three ways that they can increase the amount of exercise they get each week. Explain the purpose for daily weights for patients with congestive heart failure (CHF). Summary of the content of the teaching program How many attended the program Location of the teaching session Teaching tools that were developed (These tools should be placed in the Appendix or attached as separate documents. These should be a minimum of one teaching tool that was developed for this project.) Evaluation of the Teaching Project (20%) What barriers were encountered and how were they overcome? Results of the program evaluation (The evaluation tool should be included in the Appendix.) Other types of feedback from mentor/preceptor colleagues or attendees. What did you learn from doing this teaching project? If you could repeat this project what would you do differently?
