Explain the purpose for daily weights for patients with congestive heart failure (CHF). Summary of the content of the teaching program How many attended the program Location of the teaching session Teaching tools that were developed (These tools should be placed in the Appendix or attached as separate documents. These should be a minimum of one teaching tool that was developed for this project.) Evaluation of the Teaching Project (20%) What barriers were encountered and how were they overcome? Results of the program evaluation (The evaluation tool should be included in the Appendix.) Other types of feedback from mentor/preceptor colleagues or attendees. What did you learn from doing this teaching project? If you could repeat this project what would you do differently?
