Hypothetical Scenario: Inaccuracy in Medication Documentation

Examination of Data:

• A complete medication must include Client’s full name, the date and the time of the , the name of the medication, the ed dosage, and the form of the
medication, the route of administration, the time or frequency of administration, and the signature of the ing physician or licensed independent practitioner’s
signature

Data Access/Collection:

• Medication data were collected during an interview by direct observation using a computer-based medication log. Patient’s all medications record which he or she takes
on a regular schedule, like every day or every week. This will include prescription and nonprescription medications, over-the-counter medicines, vitamins, and herbal and
alternative medicines. “Researchers indicated that up to 26% of errors can be caused by mistakes in data entry and misinterpretation of the result in the original document
and source-to-database error rates are highly dependent on the amount of structured data collection in the clinical setting” (Naziyok et al., 2017)

Knowledge Derived from Data:

• Internal Validity- The common errors are
– Medications with similar names or similar packaging
– Medications that are not commonly used or prescribed
– Commonly used medications to which many patients are allergic
– Medications that require testing to ensure proper

• Bar-coded medication administration (BCMA) is promoted as the most effective way to reduce administration errors and is being implemented widely.

Colleague’s Responses:

• In using the patient’s medical record, we can look through past providers and staff notes of those who have worked with the patient in the past. When a nurse is reporting
findings to the provider, it is important for us to document these behaviors to avoid this reaction in the future. Reviewing and utilizing this information is helpful to

• Taiwan has been using PharmaCloud, a system that notifies providers about all medications that a patient has been prescribed. The purpose of PharmaCloud is to increase
medication safety by reducing the number of duplicated medications prescribed from different hospitals or physician clinics (Aloys Kenyoru)

• The benefits of EMR stems from the elimination of handwritten paper-based charting which obviously falls-short of electronic charting due to issues like caregivers
