Administration

Prepare the medication:

FOR AMPULE:

Tap neck lightly and quickly to force medication into the
ampule.

Wrap neck with gauze, snap off top toward self. If not
prescored, disinfect the file and neck of an ampule with cotton
ball with alcohol. Protect your fingers with OS and file the neck
of the ampule to facilitate easy opening. Wipe off dust with cotton
ball and wrap neck with gauze then break the ampule away from
yourself.

Hold ampule with bottom higher and aspirate the needed amount
of medication into the syringe by gently pulling back on the
plunger. Keep needle tip below surface of the liquid.

Tap syringe barrel to dislodge air bubbles toward needle. Draw
back plunger slightly and push plunger upward to eject air
bubbles.

Place the syringe on top of the medication ticket on the
medication tray.

FOR VIAL LIQUID FORM:

Alcoholize file. Break steel with file. Wipe the rubber top
with cotton ball soaked with alcohol.

Change the injecting needle with the withdrawal needle.

Use withdrawal needle to withdraw the medication from the vial.
Without contaminating the plunger, inject air into the vial equal
to the amount to be withdrawn. Keep needle higher than the
solution.

Hold vial with bottom higher and withdraw the desired amount of
drug. Keep tip of the needle below fluid level.

Tap syringe barrel to dislodge. Draw back plunger slightly and
push plunger upward to eject air bubbles. Do not eject fluid.
Change withdrawing needle with injecting needle.

Place the syringe on top of the medication ticket on the
medication tray.

FOR VIAL POWDERED FORM:

Alcoholize file. Break steel with file. Wipe the rubber top
with cotton ball soaked with alcohol.

Change the injecting needle with the withdrawal needle.

Disinfect the rubber top of the vial with water for
injection/distilled water using the cotton ball soaked with
alcohol. Use withdrawal needle to withdraw the desired amount of
water for injection/distilled water from the vial. Without
contaminating the plunger, inject air into the vial equal to the
amount to be withdrawn. Keep needle higher than the solution.

Disinfect the rubber top of the vial with powder using cotton
ball soaked with alcohol.

Inject the water for injection/distilled water into the powder
vial. Remove the needle and the syringe.

Agitate the mixture by inverting or rolling the vial – rolling
vial will prevent air bubbles to form in the medication.

Disinfect the vial again with cotton ball soaked with
alcohol.

Withdraw the desired amount of solution.

Tap syringe barrel to dislodge. Draw back plunger slightly and
push plunger upward to eject air bubbles. Do not eject fluid.
Change the withdrawing needle with injecting needle.

Place the syringe on top of the medication ticket on the
medication tray.

PREPARATION FOR INTRADERMAL
INJECTION:

Use tuberculin syringe/ 1 cc syringe. Change the injecting
needle with withdrawal needle.

Disinfect the rubber top of the vial with water for
injection/distilled water using the cotton ball soaked with
alcohol. Use withdrawal needle to withdraw 0.9 ml of water
for injection/distilled water from the vial. Without
contaminating the plunger, inject air into the vial equal to the
amount to be withdrawn. Keep needle higher than the solution.

Disinfect the rubber top of the vial with medication using the
cotton ball soaked with alcohol. Using the same syringe and needle,
withdraw 0.1 ml of the medication.

Tap syringe barrel to dislodge. Draw back plunger slightly and
push plunger upward to eject air bubbles. Do not eject fluid.
Change the withdrawing needle with injecting needle.

Place the syringe on top of the medication ticket on the
medication tray.

Check the medication card with the patient’s name.

Identify the client. Provide privacy and explain the procedure
to the client. Assist patient to a comfortable position.

10. Select, locate and clean the
site.
      a. Site
must be free of lesions, tenderness, swelling, hardness  
or localize inflammation and not frequently used.
      b.
Locate site for injection.

Clean site for injection with antiseptic swab, wiping from the
center of the site and rotating outward.

FOR INTRAMUSCULAR INJECTION:

Prepare the syringe for injection. Remove the needle cap
straight off. Expel the air from the syringe.

Using the non-dominant hand, spread or pinch the muscle.

Insert the needle quickly at a 90-degree angle to client’s skin
surface.

Aspirate by holding the barrel of the syringe steady with the
non-dominant hand by pulling back the plunger with the dominant
hand. If blood appears in the syringe, remove needle, dispose
syringe and prepare new medication.

If bloodless, inject the medication steadily and slowly holding
the syringe steady.

Withdraw the needle quickly while pressing antiseptic swab
above the site.

Place the used syringe on top of the tray. Do not recap
needles. Dispose of syringe and needle in sharps container. Turn
the medication card downward.

Gently massage site or apply slight pressure to the site.

Readjust the patient’s clothing.

Check the site before leaving the patient.

FOR INTRADERMAL INJECTION:

Prepare the syringe for injection. Remove the needle cap
straight off.

Hold syringe with dominant hand. Using the non-dominant hand,
grasp the client’s dorsal forearm and gently pull the skin taut on
ventral forearm.

Hold syringe at 10-15 degrees angle, gently insert needle,
bevel up until dermis barely covers bevel.

Stabilize needle, then inject slowly 3-5 seconds. Observe
development of a wheal or bleb.

Withdraw needle. Do not wipe or massage site.

Observe for signs of allergic reaction. Draw a circle around
the

perimeter of the wheal with a pen.

30 minutes after skin testing.

Do not recap needle. Dispose of syringe and needle in sharps
container.
